About the Session

As healthcare continues to shift to value-based care for improved outcomes and lower costs, providers turn to health network strategies to manage the health of patients across a complex and diverse technology landscape. Geisinger Health System’s care redesign project focused on the utilization of robust intelligence and analytical capabilities to bring together big data for its patients and health plan members. This session will focus on the sustainment of success achieved by Geisinger Health, utilizing their care redesign model geared toward continuous clinical improvement and operational efficiencies. Geisinger Health and Keystone ACO aligned internal resources to build, maintain, and manage in-house clinical registries, close gaps in care, identify high-risk patients, and manage a scalable technological environment to support the needs of the consumer and the provider.
