In the Las Vegas market, UnitedHealthcare and DispatchHealth collaborated to develop a new program to address high ED and inpatient utilization with members of UHC’s commercial and Medicaid products. By providing an in-home alternative for same day care for non-time sensitive illnesses and injuries, the Plan Directed Visit Program has reduced the incidence of unnecessary ED visits. Likewise, by identifying members who have a history of high ED utilization or who have chronic conditions known to drive ED utilization, this innovative program provides proactive home visits from a clinical team for assessment and education on non-ED alternatives for care when exacerbations occur. During this seminar, faculty will discuss the data and problems related to unnecessary ED and inpatient utilization; describe the care model and infrastructure that support the Plan Directed Visit Program; and share the outcomes since this program was implemented.
