About the Session

This session will present an overview of the social determinants of health as defined by the Centers for Disease Control and Prevention’s Healthy People 2020: economic stability, education, social and community context, health and healthcare, and neighborhood and built environment. The presenters will describe how addressing these social determinants is part of the heritage of many nonprofit healthcare organizations. You will review a framework for the role of healthcare providers in addressing social factors at the patient care level, in the operations of our organizations and in working with our communities.

Learning Objectives:

  • Describe the role of hospitals’ patient care and operations in addressing the social determinants of health and health equity.
  • Describe how hospitals can be at the table in community efforts to address social determinants of health.
