About the Session

Hospitals and health systems are continually working on ways to improve patient outcomes and experience while maintaining costs. This session will outline how data and analytics are more than just tools to measure and monitor performance—they’re strategic tools to improve patient care. The presenters will evaluate hospital, ED, urgent care and physician practices served nationwide by Vituity physician leaders and highlight how leveraging hospital systems’ full data sets can point to measurable actions to improve performance, outcomes and cost. You will take away actionable steps to take in your organization to achieve improved patient satisfaction. Such steps include data sharing among all healthcare contacts; access to patient query records; minimization of meaningless data that adds to the “noise”; breaking down of silos and building of integrated teams that promote collaboration and communication along the continuum; and availability of timely, accurate data to inform efficient treatment decisions.

Learning Objectives:

  • Learn how two approaches to care utilizing data in different ways can result in vastly different outcomes, performance measurements and patient experiences.
  • Explore best practices for creating integrated programs that utilize “right time” approaches to technology to improve outcomes, reimbursement and performance.
