About the Session

This session will introduce you to palliative care, an approach to caring for the seriously ill that uses specific communication and symptom management skills and provides an added layer of support as patients and families pursue treatment. You will explore how health systems are using palliative care processes and resources to generate revenue under value-based payment and how some payers are using these processes in quality incentive programs. The presenters also will review efforts at Bon Secours Health System to transform care for the seriously ill. This case study will highlight specific process changes used, including how to train clinical staff in communication skills, symptom assessment skills and symptom management skills, and how to define triggers for specialty palliative care consultation and build them into the EHR. You also will hear advice on how to integrate a specialty palliative care team in inpatient and ambulatory settings.
