17H - Intelligent Continuous Integrated Care (ICONIC): Clinical Innovation Through Transitions of Care

ICONIC is the future. Our current care delivery models are unsustainable, and we must innovate to meet the needs of our patients and our care teams. To achieve that, every healthcare organization will be looking to create an ecosystem of care with enhanced data, AI expertise, and blended (virtual and in-person) care teams. Healthcare executives and operational leaders will play an essential role in addressing existing problems.

ICONIC was established as a framework, spanning the full continuum of care across acuity levels within the hospital, through transitions to and from peri-hospital care settings, and in coordination with outpatient primary and specialty care practices. ICONIC’s goal is to provide continuous care across care settings. Organizationally, ICONIC works to prioritize and accelerate practice transformation by providing the tools and resources needed for multiple practices to create ICONIC models of care.

This session will focus on the development of the overarching ICONIC framework, with an in-depth discussion of the inpatient project portfolio and measurement and reporting (defining value) workgroup. Every topic will be covered within a now, near and far construct to demonstrate strategic alignment, team-based decision-making, and reflection on the lessons learned from the two years of framework implementation.

  • Develop a robust change management and communication strategy to demonstrate the value of transformative work and transparently share failures and accomplishments.
