We look forward to your participation in person at the 2022 Congress on Healthcare Leadership. As we are striving to make your experience as safe as possible, all attendees, faculty, exhibitors, staff and guest must submit their vaccination information and complete a survey through the CLEAR Health Pass.

Congress participants, faculty and exhibitors will be asked to show their green or gold status before they are able to pick up their badge and registration materials. Congress guests will need to show their green or gold status through the CLEAR Health Pass app before entering the event they are attending.

To Get Started

  • Download the CLEAR app and enroll for free here or scan the QR code and tap the white Health Pass tile
  • Select ‘Have a code’ and enter the code ACHE2022
  • Easily enroll or verify your identity with a quick selfie
  • Follow the prompts to verify your vaccination status and complete the survey

Ways to Upload Proof of Vaccination

There are three ways to provide your proof of vaccination through the CLEAR Health Pass app:

CDC Card Upload (Recommended)

  • When prompted to add your vaccination, select “Add Your Vaccination Card” to upload a photo of your CDC Vaccination Card.
  • Take a photo of your vaccination card and confirm your vaccine information.
    • Please note, the photo of your CDC card is used to generate a green Health Pass. You will not be able to access it again after it is taken.

Digitally Link

  • Select your vaccine provider in the menu or by searching ‘Other Providers.” Log in to your provider patient portal and follow instructions to securely link your account with CLEAR. The CLEAR app is integrated with select healthcare providers and pharmacies.
    • If you are unable to find your provider in search, CLEAR may not be able to link with them at this time.
    • Check that your COVID-19 vaccine appears in your provider patient portal, if not we recommend you reach out to your provider directly.
  • Your results will link automatically, and it may take up to 1 hour to verify your results within Health Pass. Do not try to resync your results.


  • Tap “SMART QR Code” and follow the prompts to upload your QR code. Once confirmed, proceed to open your Health Pass.

Helpful Tips

  • What does green or gold status mean?
    • GREEN = vaccinated and boosted AND passed health survey
    • GOLD = vaccinated but not boosted AND passed health survey
  • For the most efficient entry possible, all attendees should sign up for CLEAR and complete their health screening before arriving at Congress.
  • Before you arrive, reopen the Health Pass tile and produce your pass. Green is good to go! Be ready to show for seamless arrival.
  • Trust and transparency are CLEAR’s top priority, and with Health Pass, users are always in control of their health information.
