To make meaningful strides in improving health outcomes, organizations must adopt a systems-level approach that aligns clinical excellence with effective community-based strategies.
This session introduces the American Hospital Association’s Care Delivery Transformation Framework as a practical tool for aligning clinical care, community partnerships and operational priorities. Participants will explore real-world examples of health systems that have improved health outcomes through integrated approaches and engage in a brief design thinking exercise to develop strategies tailored to their own organizations.
