Improving health equity in patient populations is a high priority for many hospitals and health systems, as eliminating health disparities can improve patient engagement, lead to better health outcomes, decrease readmissions and improve performance in value-based care. While many organizations have advanced in their addressing of healthcare disparities, progress can be slow, and there are many barriers to change and improvement, including leadership support, funding data collection and reporting, connecting health equity with quality performance and patient loyalty/retention, and establishing KPIs and ROI. Different types of providers—such as hospitals/health systems or physician services groups—will have different paths to improving health equity in their organizations. In this session, experts in the health equity space will share how organizations can determine where they are in their journey to health equity, identify the next steps toward achieving their goal, and relate strategies for overcoming challenges and barriers to get there.
