    Patrick R. Young serves as president of Population Health at Hackensack Meridian Health (HMH), where he drives the health system's population health initiatives and spearheads strategic reforms in healthcare delivery. In this role, he leads HMH’s Population Health division, overseeing the Medicare Shared Savings Accountable Care Organization (ACO), the clinically integrated network Hackensack Meridian Health Partners, managed care, and health insurance partnerships.

    Under Patrick's leadership, HMH’s clinically integrated network has expanded significantly, growing from under 800 providers in 2015 to over 4,100 today. His focus on strategic initiatives—such as care coordination, managed care contracting and partnerships with health insurers—has propelled HMH’s population health efforts forward and earned the system notable achievements in value-based programs. Notably, HMH’s participation in Medicare’s Comprehensive Care for Joint Replacement (CJR) program has saved the system over $10 million. Additionally, the HMH ACO, which ranks among the nation’s top for savings, now serves close to 60,000 beneficiaries and consistently delivers substantial savings across its integrated care initiatives.

    Patrick's innovative leadership extends to transformative projects such as Hackensack’s “Hospital From Home” initiative, a pioneering model of hospital-level care delivered at home through a blend of virtual monitoring and in-person visits, setting a new standard in patient-centered care.

    Beyond his work at HMH, Patrick serves on the Global Innovation Hub for Value in Healthcare, an initiative of the World Economic Forum. In this role, he collaborates with global leaders to accelerate the adoption of value-based healthcare models worldwide, fostering partnerships and advancing cross-industry learning to scale transformative healthcare solutions.

    Patrick's commitment to improving healthcare outcomes and reducing costs has positioned him as a leader in value-based care, dedicated to advancing accessible, high-quality, and sustainable healthcare for communities served by Hackensack Meridian Health.
