Kristin Cannon, Vice President, Pharmacy Population Health, SSM Health has been practicing in health system pharmacy for over 30 years. She completed her B.S. Pharmacy degree at the University of Wisconsin School of Pharmacy and her MBA from the New England College. Her clinical career was forged as a pediatric pharmacist at Children’s Hospital-Minneapolis before transitioning into management as the Minneapolis campus pharmacy manager. In 2001, she accepted her first pharmacy director position and relocated to Wisconsin to open Aurora Health Care’s for-profit hospital in Green Bay, WI. Kristin spent the next 20 years with Advocate Aurora Health Care in director, senior director, and vice president roles within health system pharmacy operations. In 2022, she joined SSM Health as the WI Region Pharmacy Vice President and quickly pivoted to the system Pharmacy Population Health Vice President role. Kristin is responsible for the community pharmacy, ambulatory pharmacy, pop health, pharmacy access, home infusion, and LTC pharmacy teams across the SSM Health system. She also has matrix accountability for the SSM Health specialty pharmacy and the SSM Health pharmacy risk pool services.
