Population Health
This session will feature best practices, explore innovative/automated solutions and help healthcare leaders develop actionable plans to implement population health initiatives in their organizations. An automated approach to population health is needed in the United States, with a focus on at-risk populations, and leaders should be equipped with data-driven strategies to develop infrastructure and identify and address population health needs. Attendees will explore how automation can streamline workflows, improve care coordination and free up time for patient interaction and discuss best practices for proactive outreach programs to promote preventive care and early intervention, as well as strategies to bridge healthcare disparities and ensure equitable access to preventive care for vulnerable populations.
