Drawing from lessons learned during the COVID-19 pandemic, Penn Medicine has created a blueprint for meeting patients when and where they need care. In this approach, care is delivered across four sites: in a hospital, at a multispecialty center, in the home or through telehealth. With this model, Penn Medicine is increasing access to care, encouraging treatment adherence and enhancing patients' comfort. For example, more than 700,000 appointments have been held in private residences since last year, which also include visits from the Cancer Care at Home program. The latter has shifted about 50% of Penn Medicine–administered infusions and chemotherapy treatments to the home. Some programs are specifically designed to transition care from the hospital setting to home; others seek to reduce hospital readmissions; and several reach directly into the community through mobile mammography, colon cancer screenings, health fairs and more. These combined approaches have led to better patient experiences, reduced hospitalizations, shorter inpatient stays and decreased health costs, all of which ultimately help people live longer, healthier lives.
