Population Health
Understanding value-based care starts with embracing how the shared relationship to improve the community's heath starts with those who have accepted the privilege to provide health services to the community—the health plans and the healthcare providers. Participants will gain the confidence to connect with their health plans to discuss opportunities to embrace value-based care metrics to collectively improve the care in the community. Leaders will understand the value in partnering with each other and investing in health programs to meet the community needs (identified together with the health plans and providers) to benefit long-term community health. Shared metrics, most publicly reported, will kick off the conversations. When all providers work with their patients' best interest solidly in the center, the next level of patient-centered care can be achieved.
